Pet Name *: Pet Breed *: Gender *: Age *: Color *: Weight *: I, the undersigned, do hereby certify that I am the owner (or duly Authorized agent for the owner) of the pet described above. In being the owner or agent for this animal, I do hereby give Sunset Veterinary Services and Representatives full and complete authority to preform euthanasia services for the said pet in what ever manner Sunset Veterinary Services and representatives shall deem fit. I do also certify that the said pet has not bitten any person or animal during the last 10 days, and to the best of my knowledge has not been exposed to rabies. I do here by forever release Sunset Veterinary Services its agents and representatives from any and all liability for so euthanizing said pet. Arrangements for the aftercare will be based on the wishes of the owner or agent for the owner and documented below.

Aftercare Request

I choose to retain my pet for burial or to set up cremation myself. I choose Private Cremation at Horizon Pet Cremation (Cremains will be returned) I choose Communal/General Cremation at Horizon Pet Cremation (Cremains will not be returned.
